Instructions for use

Benign prostatic hyperplasia.

Method of administration and dosage

The recommended dose is 8 mg once daily, taken with a meal, preferably at the same time each day. Swallow the capsule whole, preferably with a glass of water. When treating patients with moderate renal impairment (creatinine clearance 30–50 mL/min), it is recommended to administer a dose of 4 mg/day during the first week; if the patient tolerates the medication well, the dose may then be increased to 8 mg/day. Silodosin is not recommended in patients with severe renal impairment.

Special instructions and precautions for use

Use with caution in patients with moderate renal impairment (creatinine clearance 30–50 mL/min). As with other alpha-1 adrenergic blockers, treatment with silodosin may cause a decrease in blood pressure and orthostatic hypotension. At the first signs of orthostatic hypotension (dizziness, weakness), the patient should sit or lie down and remain in that position until the symptoms of orthostatic hypotension subside. Because benign prostatic hyperplasia and prostate cancer share similar symptoms and may coexist, patients with suspected benign prostatic hyperplasia must undergo testing to rule out prostate cancer before this medication is prescribed. Patients who are taking or have taken alpha-1 adrenergic blockers may experience “floppy” iris syndrome during cataract surgery, which can lead to complications during the procedure. Treatment with alpha-1 adrenergic blockers must be discontinued 1–2 weeks prior to such surgery. Patients scheduled for cataract surgery should not begin treatment with silodosin. Effects on the ability to drive and operate machinery Symptoms associated with orthostatic hypotension (e.g., dizziness) may occur; therefore, patients should refrain from driving or engaging in other potentially hazardous activities until their individual tolerance to silodosin has been established.

Contraindications

Severe renal impairment (creatinine clearance <30 mL/min); severe hepatic impairment; children and adolescents under 18 years of age; hypersensitivity to silodosin.
