Tadalafil

Erectile dysfunction; lower urinary tract symptoms in patients with benign prostatic hyperplasia (for the 5 mg dose); erectile dysfunction in patients with lower urinary tract symptoms associated with benign prostatic hyperplasia.

Take orally. For erectile dysfunction: For erectile dysfunction: For sexually active patients (more than twice a week), the recommended dosage is 5 mg once daily, taken at the same time each day, regardless of mealtimes. The daily dose may be reduced to 2.5 mg depending on individual sensitivity. For patients who are not sexually active (less than twice a week), the recommended dose is 10 mg taken before anticipated sexual activity, regardless of meals. For patients in whom the 10-mg dose is not effective, a 20-mg dose is used. The medication should be taken at least 16 minutes before anticipated sexual activity. Patients may attempt sexual intercourse at any time within 36 hours of taking the medication in order to determine the optimal time for the medication to take effect. The recommended maximum daily dose of the medication is 20 mg. The maximum recommended dosage is once daily. The 10 mg and 20 mg doses are taken immediately before sexual activity and are not recommended for daily use. Patients with benign prostatic hyperplasia (BPH) or ED/BPH. The recommended dose for daily use (once daily) is 5 mg; the medication should be taken at approximately the same time each day, regardless of when sexual activity occurs. The duration of treatment is determined by the doctor on a case-by-case basis. No dose adjustment is required in patients with mild renal impairment (creatinine clearance 51–80 mL/min) or moderate renal impairment (creatinine clearance 31–50 mL/min). The use of this medication is contraindicated in patients with severe renal insufficiency (creatinine clearance <30 mL/min and on hemodialysis).

Sexual activity poses a potential risk to patients with cardiovascular disease; therefore, treatment for erectile dysfunction—including with tadalafil—should not be administered to men with heart conditions for which sexual activity is not recommended. There have been reports of priapism associated with the use of PDE5 inhibitors, including tadalafil. Patients should be advised to seek immediate medical attention if they experience an erection lasting 4 hours or longer. Delayed treatment of priapism leads to damage to the tissues of the penis, which can result in irreversible erectile dysfunction. The safety and efficacy of combining tadalafil with other PDE5 inhibitors and other treatments for erectile dysfunction have not been studied; therefore, the use of such combinations is not recommended. Like other PDE5 inhibitors, tadalafil has systemic vasodilatory properties, which may lead to a transient decrease in blood pressure. Before prescribing this medication, the doctor must carefully consider whether patients with cardiovascular disease might be at risk of adverse effects due to its vasodilatory properties. Non-arteritic anterior ischemic optic neuropathy (NAION) is a cause of vision impairment, including complete vision loss. There have been rare postmarketing reports of cases of NAION development that were temporally associated with the use of PDE5 inhibitors. An analysis of data on the intermittent use of PDE5 inhibitors for 1 to 4 days in men with erectile dysfunction suggests an increased risk of acute NAION. If a patient experiences sudden vision loss, the doctor should advise them to stop taking tadalafil and seek medical attention. The doctor should also inform patients that the risk of recurrent NAION is higher in patients who have previously had NAION. Patients with a suspected diagnosis of BPH should undergo testing to rule out prostate cancer. The efficacy of DINAMICO LONG in patients who have undergone pelvic surgery or radical prostatectomy without preservation of the neurovascular bundles is unknown. Effects on the ability to drive and operate machinery Although the incidence of dizziness is similar with placebo and tadalafil, caution should be exercised when driving or engaging in other potentially hazardous activities that require increased concentration and psychomotor speed during treatment.

Hypersensitivity to tadalafil or any other ingredient in the medication; use of medications containing any organic nitrates; age under 18; contraindications to sexual activity in patients with cardiovascular diseases (myocardial infarction within the last 90 days, unstable angina, or the onset of an angina attack during sexual intercourse, chronic heart failure class II or higher according to the NYHA classification within the last 6 months, uncontrolled arrhythmias, arterial hypotension (blood pressure less than 90/50 mmHg), uncontrolled arterial hypertension, ischemic stroke within the last 6 months); vision loss due to the development of non-arteritic anterior ischemic optic neuropathy (NAION) (regardless of a connection with prior use of PDE5 inhibitors); concomitant use with doxazosin, other PDE5 inhibitors, other treatments for erectile dysfunction, or guanylate cyclase stimulators such as riociguat; severe renal impairment (creatinine clearance <30 mL/min); lactase deficiency, lactose intolerance, glucose-galactose malabsorption.
